Calea Discharge and Resource Planning Team

This important team are the champions of effective patient discharge and support patients being set up in their own home as efficiently as possible. 

A vital part of the team is the discharge co-ordinators who liaise closely with hospital teams, Calea internal teams and the patients themselves. They are the first point of contact for referral requests from NHS hospitals. As soon as a referral is received, they will ascertain if a support package can be provided based on the patient's location, type of nutrition and nursing support required.  Local nursing capacity and compounding capacity will  be checked to ensure the patient’s needs can be met and depending on this, a patient may be able to be automatically accepted; otherwise the discharge coordinators will liaise with a Calea resource planner. They are also the first Calea contact a patient will have, as pre-discharge assessment in hospital, home risk assessment and nurse attendance for installation are arranged via the discharge coordinators.

The resource planners also work tirelessly to ensure the right nurse gets to the right patient at the right time. Their focus is patient and nurse safety and well being to support the provision of a high quality service, and prioritise ensuring an effective discharge for each patient, whilst being as responsive as possible to NHS customers.
